Start the appointment with the treatment, not the form
Intake and consent are completed in the waiting room.
Clinic Intake & Consent Engine
Patients complete intake forms and sign consent on a tablet in the waiting room, so the appointment starts with the treatment.
No card · 2 minutes
The first part of every appointment goes on paperwork.
Signed consent forms sit in a filing cabinet we can't search.

Everyday problems of the trade, in the words we hear them. Not quotes from named customers.

Yes. You can export your forms, signed consents and patient records at any time. Your data is yours, not ours.
Clinic Intake & Consent Engine is built with UK data protection in mind. You stay the controller of the data you hold about patients, and ApaOne acts as your processor. Our privacy notice and the Technology page explain how we handle and protect it.
No long contract. Clinic Intake & Consent Engine is paid monthly and you can cancel any time. There is no lock-in.
No. We do the set-up for you and load your intake forms and consent wording. Expect days, not months, and an afternoon of your time to check it over.
It can happen. We shut down products that don't earn their place, and we say so up front. You would get notice before anything changes, and a full export of your forms, signed consents and patient records.
The product is being built so that each treatment has its own form and consent wording, and that wording is yours. What a client must be told about a treatment is for your practitioner to write and your insurer to review. We would rather give you a clear structure and leave the clinical content with the people qualified to write it. Tell us on the waitlist which treatments you would set up first.
Ask your insurer, and ask in writing. UK law makes electronic signatures admissible in evidence, but insurers can set their own requirements for the records they expect to see. The product is being designed to store each signed form against the client with its date. It is at the Validating stage, so now is a good time to tell us what your insurer asks for.
Some clients will always prefer paper or need help with a screen, and a clinic should be able to offer that. The product is designed around clients completing forms themselves on a tablet in the waiting room. How paper forms and assisted completion should fit alongside that is one of the things we are asking clinics about during validation. Join the waitlist and tell us how often it comes up for you.
No software can do that by itself. Compliance comes from your decisions: your lawful basis, your privacy notice, your retention periods and who you let see records. The product is being designed with UK GDPR in mind, because these forms hold health information. It is built to remove weak points such as clipboards at the desk and forms nobody can find. The duties remain with the clinic.
It is designed around aesthetics clinics and small private clinics in the UK, where each treatment needs its own intake and consent form before the appointment starts. If your clinic fits that pattern, it may suit you. If you need full clinical notes, prescribing or diagnostic records, this is not that product. We would rather say so now than have you find out later.
Your sign-up is also a vote: it tells us this product is worth building next. If it doesn't earn its place, we'll tell you.
